About Us

Our History

Since 1989, families dealing with Alzheimer’s, disabilities, extended recovery periods, or those who just want to ensure a loved one attends their appointments have all relied on Park Avenue Home Care. We want to help you achieve the vibrantly healthy life you and your family deserve.

Park Avenue Home Care believes that one’s comfort and independence are vitally important to one’s well-being. That’s why we take a holistic approach to healing. We evaluate each client’s circumstances—from case history to environment, ongoing requirements to Rx and physician recommendations—before devising a custom plan of care. So whether you need basic, day-to-day assistance or a more advanced program, Park Avenue Home Care will take every variable into account.

Our Mission

As a Licensed Home Care Services Agency (LHSCA), we’ll partner with you to make sure your loved one stays as active, autonomous and engaged with their surroundings as ever. And regardless of the type of plan involved, from personal care (such as grooming) to patient care (such as light housekeeping), or expert guidance and advocacy for family members who’ve taken on the role of caregiver, our expert staff will work to safeguard you and your loved one’s peace of mind.

When you choose Park Avenue, you’re electing to experience the confidence that comes from knowing your loved one is in the best possible hands.
